At the end of this session delegates should be able to appreciate that accidents have both direct and indirect causes. The delegates should understand the domino theory of accident causation.

Causes and Consequences of Accidents

Explain the domino theory

Show slide Accident causation sequence which shows the domino theory. Explain how accidents are caused according to this theory, ie that is there is a progression over time with each event preceding another event until the accident is caused. If the first domino falls, the others must follow unless action is taken.
