Objectives

At the end of this session delegates should understand that accidents are largely preventable, but that once an accident chain starts then the outcome is random. They should also understand that blaming an individual for an accident is pointless.

Examples of How Accidents Happen

Explore examples of accidents

To illustrate typical accidents, show either:

  • Slide Ben is walking downstairs when he slips for administration areas, or

  • Slide Bill is using a forklift truck when the brakes fail for industrial situations.

In the case of the first example, explain that Ben slips on coffee on the stairs.
