The first priority for care providers is to keep their service users safe from any form of abuse, which can be carried out in different ways as a single act or repeated over time. Unsafe practices from, say, poor standards of nutrition, hygiene or infection control, neglect of pressure sores or incorrect moving and transferring methods could also be acts of abuse. They would show that the service is failing its users in its duty of care to keep them safe.
