The appropriate “Competent Person” should observe the activity that is to be assessed and record the assessment in a suitable and permanent form. It is good practice to involve those carrying out the activity on a regular basis in the risk assessment process. Each assessment must be dated and then signed off, a review date should also be included. Any actions identified should be implemented.

Example of a Risk Assessment

The following elements should be included in the risk assessment:

  • Place where assessment was carried out1

  • Date of assessment

  • Date activity completed

  • Name of the assessor

  • Details of the activity

  • The hazards

  • Degree of risk2

  • Nature of risk

  • Likelihood of the risk3

  • The risk rating
