This policy sets out the agency’s approach to its service users who have or intend to have an advance care plan (ACP).
The agency will always check if its service users have or intend to have an ACP which will set out the care they want to receive if their physical and mental health declines to the point where they lose mental capacity.
During the COVID-19 pandemic this agency believes services users having an advanced care plan (ACP) in place or being able to review existing ACPs is of high importance. This incorporates discussion around how COVID-19 may cause service users to become critically unwell.
