The government and fire and rescue services are failing to learn lessons from the deaths of four Warwickshire firefighters in a warehouse blaze in 2007, according to a scathing incident report by the Fire Brigades Union.

Ian Reid, 47, John Averis, 27, Ashley Stephens, 20, and Darren Yates-Badley, 24, firefighters for Warwickshire Fire and Rescue Service (WFRS), died having been sent in to a blazing vegetable packing warehouse in Atherstone-on-Stour on 2 November 2007.

According to the FBU twice as many firefighters have lost their lives over the last decade as in the previous 10 years.
